Saving Lives with University of Chicago Medicine
Illinois Care CONTINUUM, led by University of Chicago Medicine, is a proprietary collaborative website application for hospital systems to connect with their partnering clinics for follow up specialty care. It encompasses a complete patient flow management – from testing and patient outreach on community level, through partner clinics, to partner hospitals for specialty care. Working with a large number of stakeholders, it was imperative that ActiveColor follows a clear process – starting with strategic workshops, performing deep research, and establishing clear KPIs.
